Get Assessed
Contact UnaCare and learn more about qualifying for services through the New York State of Health/Medicaid. FreedomCare will be with you every step to make sure you can get the care you need from the comfort of your home and with someone you trust.
Enroll a Caregiver
We can match you with a qualified caregiver or you can recommend an individual to get the care you need. Caregiver enrollment has three parts: a health assessment, proof of identity and work authorization, and an orientation with UnaCare.
